This is a new old stock fender jazz bass control plate.  Highly polished / detailed chrome.  Great quality....it is either fender or a USA aftermarket, probably from the 1990's.   Surface shows storage wear in the form of light handling marks.  It would match to a guitar that has some light normal handling.  Back shows a tag "Slater's music".
