Measuring about 6-1/4" x 9-1/4", and 609 pages long, this hardback book is "Nuclear Physics" by Irving Kaplan. Published in Reading, Massachusetts, by Addison-Wesley Publishing Company, it has a copyright date of 1954 and a publication date of 1958, and is stated to be the fourth printing. 

It comes with the two-sided chart shown in the last two pictures, which measures 23" x 17-1/4" and is titled "General Electric Chart of the Nuclides, Knolls Atomic Power Laboratory (Operated by the General Electric Company for the Atomic Energy Commission)." It is stated to be the Fifth Edition, revised to April, 1956, and published by Addison-Wesley "with the permission of the General Electric Company." Although it is loosely laid in, the chart was published as part of the book - it is listed as "Appendix IV" in the Table of Contents.

The book is in very good and readable condition, with the binding nice and tight, but with some surface soiling and edge wear on the cover. It also has an ownership inscription on the front end paper, as well as some creased pages near the front of the book, and many pages with underlinings in ink (some of which are shown in the pictures). The chart has been folded to fit inside the book, and it also has some diagonal lines that might be hand drawn (although most of them look to be printed when viewed under a magnifying glass).
