This is a sample football program that would have been sold to high schools to print their game programs on. There's no company name, but "Design No. 212 Printed in U.S.A." is in small print on the back. Online I found a Texas high school football program from 1962 which used this form. The cover has an endearing image of a young boy in his football uniform. He's sitting with a sad expression next to a Basset Hound, with the broken window of a house in the background.  Inside is the space for the details of the game, with a Coke ad around the top and sides. There's a drawing of a cheer leader and small color photos of games. At the bottom is a chart identifying foul and procedure signals. The folded sheet opens up to measure  16 by 11 1/8 inches. The paper is lightly toned, with small tears in the center fold lines.
