I just received my order but they don't appear to be self-adhesive. Am I missing something?

the package is come with some adhesive stickers ,but If needed, please use double sided tape(tape is not include).

Does the floor need to be level to use these?

No , we laid ours over top of concrete that isn’t level in some places , you can’t even tell. You can vacuum it and all , and it won’t budge
